「자동차관리법」제30조의8제3항제1호 및 같은 법 시행규칙 제40조의22제2항에 따라 안전성능시험 시험기관 지정을 신청합니다.
The applicant hereby seeks the designation of a safety performance test institution under Article 30-8 (3) of Motor Vehicle Management Act and Article 40-22 (2) of its Enforcement Rule.

	※ 자동차 기술ㆍ안전 국제조화기구(UNECE/WP.29)에 등재된 신청인은 제1호부터 제3호까지의 서류를 첨부하지 않을 수 있습니다. (Exemption from submitting the first to the third documents may be granted to an applicant registered with the UNECE/WP.29.) 1. 다음 각 목의 시험시설 확보 관련 서류 (Information on testing facilities) 가. 시험시설명, 제조사, 모델명, 제조번호가 포함된 사진 등 (Photo shows the name of test facility, manufacturer, model, and serial No.) 나. 시험시설의 사양서(규격서), 교정성적서, 시설 점검 및 관리 이력대장 등 (Specification, calibration certificate, facility inspection and maintenance log) 다. 시험시설의 설치에 관한 서류(시험시설 내ㆍ외부 사진, 시설 배치도 등을 포함합니다) [Information of the place where the test facilities are located (photos of in/outside of test facility, and the layout of the test facilities)] 2. 다음 각 목을 모두 충족하는 시험인력 확보 관련 서류 (Information on testing personnel) : 가. 「국가기술자격법」에 따른 산업기사 이상의 자격을 소지한 사람으로서 해당 시험 분야에서 실무경력(자격 취득 전의 실무경력을 포함합니다)이 3년 이상일 것(At least 3 years of experience in the relevant field who holds a national technical qualification of Industrial Engineer or higher under the National Technical Qualifications Act) 나. 시험인력이 2명 이상일 것(At least 2 test personnel) 3. 국제표준화기구(ISO)에서 발급한 시험 및 교정기관의 적격성에 관한 일반 요구사항(ISO/IEC 17025)에 대한 인정서(ISO/IEC 17025 Certification) 또는 이에 준하는 인정서 4. 핵심장치등의 안전성능시험을 수행하기 위한 절차 및 방법 등을 기록한 업무규정 (Business regulations recording procedures, methods, etc. for conducting safety performance test) 5. 대리인이 신청하는 경우 그 위임 사실을 증명할 수 있는 서류 [A certificate of relationship with the applicant (for a proxy application)] 6. 그 밖에 국토교통부장관이 필요하다고 인정하여 고시하는 서류 (Other documents announced as necessary by the Minister of Land, Infrastructure and Transport)		사업자등록증명(주민등록번호가 제외된 사업자등록증명을 말합니다. 이하 이 서식에서 같습니다) 또는 법인 등기사항증명서(외국 시설의 경우 이에 준하는 서류를 제출해야 합니다) [Business registration certificate (not including Resident Registration Number) or a certified copy of corporation register book (In case of foreign facility, equivalent documents must be submitted)]

행정정보 공동이용 동의서 (Consent for the Use of Information)
본인은 이 건 업무 처리와 관련하여 담당 공무원이「전자정부법」 제36조제1항에 따른 행정정보의 공동이용을 통하여 위의 사업자등록증명을 확인하는 것에 동의합니다. * 담당 공무원의 확인에 동의하지 않는 경우에는 신청인이 직접 관련 서류를 제출해야 합니다. Regarding the handling of this matter, I hereby agree that the official in charge may confirm the above ‘Matter to be confirmed by the official in charge’ through the co-use of administrative information according the Article 36-1 of 「E-Government Act」. * Document to be submitted in case the above matter is not agreeable.
