「방위사업법」 제57조제2항 및 같은 법 시행령 제68조제3항에 따라 위와 같이 방산물자 및 국방과학기술의 수출 허가를 신청합니다.
I apply for export license pursuant to Section 2, Article 57 of the 「Defense Acquisition Program Act」 and
Section 3, Article 68 of the 「Enforcement Decree of the Defense Acquisition Program Act」.

수입자, 최종수하인 또는 최종사용자가 최종 목적지국가가 아닌 국가나 지역으로 허가 받은 전략물자를 재수출하지 않는다는 조건하에 「방위사업법 시행령」 제68조에 따라 위와 같이 방산물자 및 국방과학기술의 수출을 허가합니다.
On condition that the importer, the Ultimate Consignee or the End-User would not re-export the approved items to countries or regions other than the country of Ultimate Destination stated above, the undersigned hereby approves the above items to be exported in accordance with Article 68 of the 「Enforcement Decree of the Defense Acquisition Program Act」 .

2. 별지 제19호의2서식의 최종사용자 증명서 1부
3. 수출품목의 성능과 용도를 표시하는 설명자료 1부
4. 대한민국 소유의 기술을 사용하는 물자의 수출이나 해당기술을 직접 수출하는 경우에는 기술보유기관과 체결한 기술이전계약서 1부
5. 수출하는데 외국정부의 동의나 허가가 필요한 경우에는 해당 국가로부터 받은 동의서 또는 허가서 1부
6. 국방과학기술을 수출하는 경우에는 해당 기술의 수출에 따른 국내외의 파급 효과 설명서 1부
7. 수출품목의 납품일정 설명자료 1부
