「수입식품안전관리 특별법」 제6조의2제2항 및 같은 법 시행규칙 제3조의2제4항에 따라 위와 같이 인증사항의 변경을 신청합니다. I hereby apply for the changes in the certification of HACCP in accordance with Article 6-2(2) of the Special Act on Imported Food Safety Control and Article 3-2(4) of the Enforcement Rule of the same Act.

｢수입식품안전관리 특별법 시행규칙」 별표 15 제5호에 따라 인증원의 장이 정하는 수수료
Fee which is determined by the Head of the Korea Agency of HACCP Accreditation and Services(KAHAS) in accordance with attached Table 15 subparagraph 5 of the Enforcement rule of the Imported Food Safety Control

유의사항 Note

인증서를 잃어버린 경우에는 분실사유란에 그 사유를 적고 인증서는 첨부하지 않아도 됩니다.
In case of a lost certificate, please fill in the Reason for Loss of Certificate and do not need to attach a copy of certificate.
