「수입식품안전관리 특별법」 제6조의2제1항ㆍ제3항 및 같은 법 시행규칙 제3조의2제2항ㆍ제5항에 따른 식품안전관리인증기준적용업소 인증 또는 인증 유효기간의 연장을 신청합니다. I hereby apply for the certification/extension of HACCP in accordance with Article 6-2(1)ㆍ(3) of the Special Act on Imported Food Safety Control and Article 3-2(2)ㆍ(5) of the Enforcement Rule of the same Act.

1.「수입식품안전관리 특별법」제6조의2제1항에 따라 작성한 적용대상 식품별 식품안전관리인증계획서(중요관리점의 한계 기준, 모니터링 방법, 개선조치 및 검증방법을 기술한 자체 계획서 등을 말합니다)
A HACCP plan(a voluntary plan explaining critical limit, monitoring method, corrective measure and HACCP verification) for each type of food subject to HACCP in accordance with Article 6-2(1) of the Special Act on Imported Food Safety Control.

2. 수입식품안전관리인증적용업소 인증서 원본 또는 사본(법 제6조의2제3항에 따른 인증 유효기간의 연장을 신청하는 경우만 제출합니다)
The original or a copy of the certification for HACCP-applied facility (submit only when extending the validity period of the certificate in accordance with Article 6-2(3))

Fee which is determined by the Head of the Korea Agency of HACCP Accreditation and Services(KAHAS) in accordance with attached Table 15 subparagraph 5 of the Enforcement rule of the Imported Food Safety Control
