「수입식품안전관리 특별법」 제5조제1항ㆍ제2항ㆍ제7항 및 같은 법 시행규칙 제2조제1항ㆍ제3항ㆍ제4항에 따라 위와 같이 등록(변경등록 또는 유효기간 연장)을 신청합니다.
In accordance with Article 5(1)ㆍ(2)ㆍ(7) of the Special Act on Imported Food Safety Control and Article 2(1)ㆍ(3)ㆍ(4) of the Enforcement Rules of the same Act, I hereby apply for registration

첨부서류
Documents required for application

해외제조업소가 해당 수출국의 식품 등 관련 법령에 따라 허가·등록·신고 등이 되어 있음을 확인할 수 있는 국가 증빙 서류
- 해당 서류에는 업소명, 소재지 등 관련 정보가 반드시 포함되어야 하고, 해당 서류를 제출할 수 없는 경우 「수입식품안전관리 특별법 시행규칙」 별지 제1호의2서식의 해외제조업소 등록정보 확인서로 대신할 수 있습니다.
* 국가 증빙 서류를 번역하여 제출할 경우, 공증본으로 제출해야 합니다.
Documents issued by the exporting country’s competent authority certifying that the foreign food facility is authorized, registered or declared according to the country’s relevant laws and regulations regarding food products
- Such documents must include relevant information including the name and address of the foreign food facility. However, when such documents cannot be submitted, Confirmation Form of Registered Information of a Foreign Food Facility (Form 1-2) may be used instead.
* When submitting translated version of such documents, they should be notarized.

유의사항 Note

1. 거짓이나 그 밖의 부정한 방법으로 해외제조업소를 등록ㆍ변경한 것으로 인정되는 경우에는 「수입식품안전관리 특별법」 제5조의2에 따라 등록이 취소되며, 해당 해외제조업소의 등록 또는 변경등록을 신청한 수입자 등은 등록이 취소된 날부터 2년이 경과하기 전에는 제5조제1항에 따른 등록 신청을 할 수 없습니다.
If it is determined that a foreign food facility made its registration or alteration by fraud or any other improper means, its registration will be revoked according to Article 5-2 of the Special Act on Imported Food Safety Control, and application for registration or alteration of such foreign food facility made by an importer or any other related parties shall not be accepted for 2 years from the date when the revocation is determined as per 5(1) of the same Act.
2. 식품안전에 관한 관리시스템 적용 시 식품제조 과정 전반에 대한 안전성을 객관적으로 확인할 수 있는 국가 증거 서류 또는 국제적으로 통용되는 인증서류를 제출해야 합니다.
When employing the food safety management systems, documents issued by the exporting country’s competent authority or internationally accepted certification documents able to objectively verify the safety of overall food manufacturing processes must be submitted.
