| 이 증서는 「2006 해사노동협약」(이하 "협약"이라 한다)의 기준 제A5.1.3조 제7항의 목적에 따라 다음 각 호의 사항을 증명합니다. 1. 이 선박은 아래 제2호, 제3호 및 제4호에 관한 인증을 고려하여 협약 부록 A5-Ⅰ에 수록된 사항에 대하여 실행 가능한 한 합리적으로 검사되었습니다. 2. 선박소유자는 이 선박이 협약을 준수하기 위한 적절한 절차를 갖추고 있다는 것을 관할청 또는 인증검사 대행기관에게 입증하였습니다. 3. 선장은 협약의 요건과 이행책임을 숙지하고 있습니다. 4. 해사노동적합선언서를 발급받기 위해 관계 정보가 관할청 또는 인증검사 대행기관에게 제출되었습니다. This is to certify, for the purposes of Standard A5.1.3, paragraph 7, of the Convention, that: 1. this ship has been inspected, as far as reasonable and practicable, for the matters listed in Appendix A5-I to the Convention, talking into account verification of items under 2, 3 and 4 below: 2. the shipowner has demonstrated to the competent authority or recognized organization that the ship had adequate procedures to comply with the Convention: 3. the master is familiar with the requirements of the Convention and the responsibilities for implementation: and 4. relevant information has been submitted to the competent authority or recognized organization to produce a Declaration of the Maritime Labour Compliance. 이 증서는 「선원법」 제137조에 따른 검사를 수검할 것을 조건으로 년 월 일까지 유효합니다. This Certificate is valid until subject to inspections in accordance with Standards A5.1.3 and A5.1.4. 상기 제1호에 규정하는 검사의 완료일은 년 월 일입니다.
