| 1. 선박명/항공기ㆍ열차ㆍ자동차 등록번호: Name of Vessel or Registration Marks of Aircraft/Train/Car 2. 국적: Nationality 3. 선박 종류 또는 항공기형ㆍ열차ㆍ자동차의 형: Description of Vessel/Type of Aircraft/Train/Car 4. 위의 선박ㆍ항공기ㆍ열차ㆍ자동차에 대하여 검역 법규에서 정한 바에 따라 검역을 실시한 결과, 이 선박ㆍ항공기ㆍ열차ㆍ자동차를 통하여 검역감염병의 병원체가 국내에 침입할 우려가 거의 없다고 인정하여 「검역법」 제23조제1항에 따라 우선 이 증서를 발급합니다. 따라서 년 월 일까지 다음 조건을 이행하고 검역증을 발급받아야 합니다. As a result of the quarantine inspection conducted with the above mentioned vessel/ aircraft/train/car under the provisions of the Quarantine Act, it has been determined that little or no danger exists as to the possible invasion of quarantinable disease into the Republic of Korea from such vessel/aircraft/train/car. Accordingly, under the provisions of Article 23 of the Quarantine Act, it is hereby decreed that this certificate will be replaced with a new certificate of Free Pratique provided the following conditions are complied with by (day) , (month) , (year) . 다음 조건 Conditions | ||||||
