| 1. 도착 장소 및 도착 예정 일시: 운송수단이 도착할 장소의 이름과 도착 예정 일시(한국시간)를 적습니다. The Place and the date of expected arrival: Write down the place and the expected date of arrival (Declare the expected time of arrival according to korean time). 2. 출발지 및 출발 일시: 출발한 곳의 지명과 출발 일시를 적습니다. The last place of departure and the date of departure: Write down the place and the date of departure 3. 다음 도착 국가명 및 다음 도착 지명: 다음 도착할 국가 및 도착할 곳의 이름을 적습니다. The name of next destination country and port of destination: Write down the name for next destination country and port. 4. 주요 적재 화물명 및 화물을 내리는지 여부: 적재된 주요 화물명과 화물을 내리는지 여부(화물을 내리는 운송수단은 "있음"으로, 그렇지 않은 운송수단은 "없음"으로 표시합니다)를 적습니다. Major contents of cargo and Unloading cargo: Write down contents of cargo and whether the cargo is unloaded or not (If you unload cargo, write Yes. If not, write No). 5. 21일 이내 머물렀던 곳의 지명 및 일시: 도착시간으로부터 21일 이내에 머물렀던 곳의 지명 및 일시 전부(머무른 순서대로 기입해야 하며, 머물렀던 곳이 없거나 정확한 일시를 모르면 생략합니다)를 적습니다. List any ports of entry visited within the last 21 days and the dates of each visit: Write down all of the places where you stayed within 21 days from time of departure(List only if applicable and in chronological order by date; otherwise, leave blank). 6. 도착 장소에서 승선ㆍ탑승한 뒤 다음 장소로 출발하기 전 내리는 사람의 수(명): 도착 장소에서 승선ㆍ탑승한 뒤 다음 장소로 출발하기 전 내리는 사람의 수를 적습니다. The number of people boarding at the arrival point then getting off before departure for the next destination: Write down the number of people who are boarding at the arrival point and then getting off before departure for the next destination. 7. 승무원의 수 및 승무원이 내리는지 여부: 승무원의 수와 승무원이 내리는지 여부(승무원이 내리는 운송수단은 "있음"으로, 그렇지 않은 운송수단은 "없음"으로 표시합니다)를 적습니다. Number of crews and crews getting off: Write down the number of crews and whether crews are getting off or not(If crews are getting off, write Yes. If not, write No). 8. 승객의 수 및 승객이 내리는지 여부: 승객의 수와 승객이 내리는지 여부(승객이 내리는 운송수단은 "있음"으로, 그렇지 않은 운송수단은 "없음"으로 표시합니다)를 적습니다. Number of passengers and passengers getting off: Write down the number of passengers and whether passengers are getting off or not(If passengers getting off, write Yes. If not, write No). 9. 환자 및 사망자 유무: 검역감염병에 의한 환자 및 사망자 발생 유무를 적습니다. Existence of patients or dead bodies/fatalities: Write down whether patients or dead bodies/fatalities due to the quarantine infectious diseases existed or not. 10. 감염병 매개체와 병든 동물 및 화물의 검역감염병 병원체 오염 유무: 배 안에 감염을 일으키거나 질병을 전파시킬 수 있는 쥐 또는 위생해충이 서식하고 있는지 또는 서식한 흔적이 있는지 여부, 병든 동물 유무 및 화물이 검역감염병의 병원체에 오염되었거나 오염되었을 것으로 의심되는지를 적습니다. Existence of a vehicle for the communicable diseases or a sick animal and Existence of a cargo contaminated with a quarantine infectious disease agent: Write down whether there are any rats or harmful insects which can spread the diseases inhabiting the ship. Also, write down whether there are any cargo suspected of being contaminated with a quarantine infectious disease agent. 11. 선박위생관리증명서 발급 유무 및 발급 연월일: 선박위생관리에 관한 증명서의 발급 유무 및 발급 연월일을 적습니다. Ship sanitation control certificate issued and Date of issuing of ship sanitation control certificate: Write down whether ship sanitation control certificate has been issued and the date of issuance, if applicable. 12. 담당자 성명 및 담당자 전화번호: 검역조사 생략을 신청하는 담당자의 성명 및 전화번호를 적습니다. The name and telephone number of manager: Write down the name and telephone number of manager applying for exemption of quarantine inspection. 13. 생략 신청 사유: 검역조사 생략 신청을 하는 사유 및 도착 목적을 적습니다. Reason for application: Write down the reason for application for exemption of quarantine inspection(or purpose of visit). 14. 신청 일시: 검역조사 생략을 신청하는 일시를 적습니다.
