보건상태 신고 DECLARATION OF HEALTH 1. 비행기 안에 항공병 또는 사고 외의 질병으로 앓은 사람(발진, 발열, 오한, 설사 증세를 나타내는 사람을 포함합니다)이나 질병으로 사망한 사람 또는 도중에 위와 같은 증세로 인해 내린 사람을 별지 명세표에 적습니다. Persons on board with illnesses other than airsickness or the effects of accidents, (including those with symptoms such as rash, fever, chills, and diarrhea), or who died of illnesses or disembarked during the flight due to such symptoms. Provide details in the attached Schedule. 2. 비행기 안에 질병을 퍼뜨릴 수 있는 그 밖의 조건을 별지 명세표에 적습니다. Any other conditions on board which may lead to the spread of disease. Provide details in the attached Schedule. 3. 비행 중 감염병 매개체(쥐 또는 위생해충)를 발견ㆍ조치하였으면 그 내용(소독장소, 작업일, 소독방법, 사용 약제명)을 별지 명세표에 상세히 기록하고, 만약 비행 중에 아무런 감염병 매개체도 발견하거나 조치하지 않았다면 가장 최근에 해충구제 조치를 한 것을 상세히 기록합니다. If any vectors of infectious diseases (e.g., rats or other vermin) were detected and follow-up measures were taken during the flight, provide detailed information in the attached Schedule (place of disinsection, date of operation, method, and chemicals used).
