작성방법 및 유의사항
1. 입항ㆍ출항 24시간 전에는 "최초"란에, 입항ㆍ출항 확정 시에는 "최종"란에, 변경 시에는 "변경"란에, 취소 시에는 "취소"란에 각각 표시해 주시기 바랍니다. If you are filling out this form 24 hours prior to departure or arrival, mark the "Notice" box. If your arrival or departure has been confirmed, mark the "Final" box. If you are making a change, mark the "Change" box. If you are canceling, mark the "Cancel" box. 2. 5번의 「선박 계선 장소(선석, 장소)」는 선박의 접안 또는 계선 장소를 적습니다. For Section 5 「Position of the Ship (Berth or Station)」 with the pier or anchoring site of ship. 3. 6번 중 「경유지 및 경유지 출항일」은 모든 경유지를 대상으로 최초 출항지로부터 경유되는 역순으로 적습니다. For 「Stopovers」 and 「Date of Departure from Stopover」 in Section 6, provide information of every transit since first target in reverse order. 4. 7번 중 「화물명」 코드란에는 「통일상품명 및 부호체계에 관한 국제협약」에 따른 통일체계 품목분류표(HS code) 중 대표화물 두 자리 코드와 총 적재화물 중 적하ㆍ양하/위험물/환적에 해당하는 톤수를 적습니다. For 「Name of Cargo」 of Section 7, provide the two-digit HS code of representative luggage AND the tonnage of load/unload/dangerous/transshipment cargo. 5. 9번 중 ※는 국내 항 간의 승선 허가자와 동승 출항 예정 외국인의 입국 허가자 수를 적습니다. For ※ Coastal Travellers / Alien permitted landing while the ship is in Korea of Section 9, write the number of persons permitted on board between domestic ports including the number of foreign nationals permitted entry. 6. 10번의 「도착 목적」은 다음 해당 코드를 적습니다. For Section 10 「Purpose of Arrival」, select a code from the following: 01. 적하ㆍ양하 Load/Unload 02. 적하 Load 03. 양하 Unload 04. 선박 수리 Ship repair 05. 해난 Maritime distress 06. 승무원 교대 Change of crew 07. 여객 상륙 Landing flight 08. 급유 Refueling 09. 선용품 적재 Ship’s stores 99. 기타 Other 7. 11번의 「항해 구분」은 다음 코드 중 해당하는 선박만 적습니다. For Section 11 「Voyage Classification」 , select applicable code from: 01. 외국적 내항선 Foreign Coaster 02. 일시 외항 Temporary Outport 03. 내외항 자격 변경 Change of Qualification of inner/outer harbor 04. 나포선박 Ship seizure 05. 개인 항해 Personal Voyage 06. 신조선 New Ship 07. 수출 선박 Export Ship 08. 선명 변경 Change of Ship’s name 09. 폐선 예정 Ship to be abandoned 10. 남북한 출입 South-North Entry/Exit 11. 실습선 또는 관공선 School ship or Government ship 12. 원양 조업선 Pelagic fishing vessel 8. 12번의 「검역 구분」은 다음 코드를 적습니다. For Section 12 「Type of Quarantine」 select one of the following codes: 1. 승선 On board 2. 서류심사 On paper 3. 검역조사 생략 Skip Quarantine Inspection 4. 검역 완료 Quarantine completed 9. 13번의 「검역조사 생략 신청사유」는 다음 코드를 적습니다. (검역감염병 환자등과 사망자가 없는 운송수단만 검역조사 생략 신청 가능) For Section 13 「Reason for applying to skip Quarantine Inspection」, select one of the following codes: (NOTE: Only vessels without infectious disease patients or death are allowed to skip quarantine inspection) 1.운송수단이 화물을 내리지 않고, 우리나라에 일시 머무르는 목적이 다음 중 하나인 경우 Temporary stay without unloading cargo for one of the following reasons: ①급유ㆍ급수 Loading Fuel or Water, ②물품공급 Supplying Goods, ③도착ㆍ출발증명서 수령 Certificate of Arrival/Departure, ④운송수단 수리 Vessel Repair, ⑤태풍 등 기상악화 Bad Weather 2.검역감염병 환자등 및 감염병 매개체가 없는 군용운송수단 Military transportation without infectious disease vector or infectious disease patient 3.통일부장관이 요청하는 운송수단 Transportation requested by the minister of unification 4.관계 중앙행정기관의 장이 국가재난 등 긴급한 대응이 필요한 사유로 검역조사 생략을 요청한 경우 Head of central administrative agency has requested to skip quarantine inspection due to national emergency 10. 16번의 「수속 장소 및 일시」는 입항ㆍ출항 전 C.I.Q. 기관의 수속을 받으려는 예정 장소와 일시를 적습니다. For 「Date/Place of CIQ Processing」 of Section 16, provide place and date you will undergo C.I.Q. process before arrival/departure. 1. 항구명 Name of Port 2. 내항 Inner Port 3. 외항 Outer Port 11. 17번 중 「이 항구 승선ㆍ하선 승무원/승객」란은 입항 시에는 승선ㆍ하선 예정 인원을, 출항 시에는 승선ㆍ하선 인원을 각각 적습니다. For 「Number of Crew members and Passengers Embarked/Disembarked at this port」of Section 17, provide number of crew members and passengers that will be embarking/disembarking or has embarked/disembarked at this port. 12. 18번의 「선박위생 증명서」는 선박위생에 관한 증명서의 소지 여부, 증명번호, 발행기관명, 발행연월일, 증명서의 종류(관리/면제), 재검사 유무를 적습니다. 이 때, 유효기간이 지난 증명서는 소지 여부에 ‘아니오’라고 작성합니다. For「Ship Sanitation Certificate」 of Section 18, indicate whether a valid sanitation certificate is available on board, the certificate number, the name of the issuer of the certificate, issued date, certificate class, and whether a reinspection is needed. Check no if sanitation certificate has expired. 13. 19번 「비고」는 다음을 참고하여 작성합니다. For Section 19, answer the questions in consideration of the following 가. 감염병 매개체 유무: 배 안에 감염을 일으키거나 질병을 전파시킬 수 있는 쥐나 모기 또는 위생해충이 서식하고 있거나 서식한 흔적의 유무를 적습니다. Infectious disease vector on board: Check yes if there are indications of rats or mosquito or insects on board that can infect or transmit diseases. 나. 오염물건 유무: 검역감염병의 병원체에 오염되었거나 오염되었을 것으로 의심되는 물건의 유무(검역감염병이 현재 유행하고 있는 지역에서 적재한 물건이 있는 선박은 "있음"으로, 그러하지 아니한 선박은 "없음"으로 할 것)를 적습니다. Contaminated goods on board: Check yes if Write down the contaminated goods by pathogen of infectious disease of quarantine even suspected things(If the vessel has a loaded goods from the current infected area, write "Yes". If not, write "No"). 다. 위생조치 유무: 배 안에서 검역, 격리, 살균 또는 오염제거 등의 위생조치를 실시하였는지 여부 및 위생조치를 실시한 경우에는 유형, 장소, 일시를 적습니다. Sanitary measures taken: Indicate sanitary measures such as quarantine, isolation, sterilization, and decontamination has been conducted in the ship. (If yes, write type of measure, place and date.) 라. 선의의 승선 유무와 진료 여부 및 결과: 배 안에 근무하는 선의의 승선 유무 및 진료 여부를 적습니다.(진료를 하였을 경우에는 진료 결과를 포함할 것) Ship’s surgeon on board and Treatment result(If any): Indicate whether a ship’s surgeon is on board, whether treatment has been given, and result of treatment if treated.
