| 「식품ㆍ의약품분야 시험ㆍ검사 등에 관한 법률」 제8조제2항ㆍ제8조의2제3항 및 같은 법 시행규칙 제7조제2항ㆍ제8조의3제1항에 따라 위와 같이 국외시험ㆍ검사기관의 지정(재지정)을 신청합니다. I hereby apply for Foreign Testing Laboratory in accordance with Article 8(2) and 8-2(3) of Act on Testing and Inspection in the Food and Drug Industry and Article 7(2) and 8-3(1) of the Enforcement Rule of the Act. | |||||||||||

| 국외시험ㆍ검사기관 지정신청 첨부서류 Documents required for application | ||||||||||||
| 1. 시험ㆍ검사기관의 일반현황(연혁을 포함합니다) General information (including history of the laboratory, etc.) 2. 시험ㆍ검사기관의 조직 및 인력 현황(경력사항을 포함합니다) Laboratory organization and laboratory personnel (including work experiences) 3. 시험ㆍ검사 시설의 평면도 Floor plan of testing facility 4. 시험ㆍ검사 설비ㆍ장비 및 기구의 보유 현황(시험ㆍ검사실의 배치도를 포함합니다) A list of equipment necessary for testing (name of the equipment, manufacturer, model name, quantity, etc. including laboratory floor plan) 5. 시험ㆍ검사실적(지정받으려는 시험ㆍ검사 대상에 대한 최근 3년간의 시험ㆍ검사 실적). 다만, 「식품ㆍ의약품분야 시험ㆍ검사 등에 관한 법률」 제8조제1항제3호에 따른 기관은 이를 설립ㆍ운영하는 국내 식품전문 시험ㆍ검사기관의 최근 3년간의 실적으로 대신할 수 있습니다. Testing records (recent three-year records of testing fields that the laboratory is seeking to get designated). However, for laboratories located outside of Korea that are established and operated by a MFDS-designated food testing laboratory, testing records produced from the food testing laboratory can be used instead. 6. 수출국 정부가 검사기관으로 공인한 서류 사본(「식품ㆍ의약품분야 시험ㆍ검사 등에 관한 법률」 제8조제1항제2호에 해당하는 경우만 해당합니다) A copy of documents including certificate of testing laboratory issued by the government of exporting country (Only required when it conforms to Article 8 (1) 2 of Act on Testing and Inspection in the Food and Drug Industry) 7. 시험ㆍ검사성적서 서식(「식품ㆍ의약품분야 시험ㆍ검사 등에 관한 법률 시행규칙」 별지 제10호서식의 시험ㆍ검사성적서를 사용하지 않은 경우만 해당합니다) A copy of laboratory testing report form (not necessary when the laboratory testing report is issued in accordance with the form 10 prescribed in this Act) | ||||||||||||

| 1. 국외시험ㆍ검사기관 지정서 원본 Original Certificate of Testing Laboratory 2. 「식품ㆍ의약품분야 시험ㆍ검사 등에 관한 법률 시행규칙」 제7조제2항 각 호에 따른 서류(전자문서를 포함하여 변경된 사항이 있는 경우에만 제출합니다) Documents in Article 7 (2) of the Enforcement Regulation of Act on Testing and Inspection in the Food and Drug Industry, only in case of the changes 3. 국제표준화기구(ISO)에서 발급한 시험 및 교정기관의 적격성에 관한 일부 요구사항(ISO/IEC 17025) 인정서 및 직전 현장평가 결과(「식품ㆍ의약품분야 시험ㆍ검사 등에 관한 법률 시행규칙」 제8조의3제3항에 따른 현장조사 면제 대상인 경우에만 제출합니다) A copy of the accredited ISO/IEC 17025 certificate and Result document of On-site assessment to obtain and/or maintain ISO/IEC 17025 accreditation (result of the latest 2 years should be submitted, and this applies to only laboratories which are subject to the exemption of the on-site assessment in accordance with Article 8-3 (3) of the Enforcement Regulation) | ||||||||||||
