1995년「어선 선원의 훈련ㆍ자격증명 및 당직근무의 기준에 관한 국제협약」과 그 개정규정에 따라 해기사면허증이 인정된 것을 증명하는 승무자격증(ENDORSEMENT ATTESTING THE RECOGNITION OF A CERTIFICATE UNDER THE PROVISIONS OF THE INTERNATIONAL CONVENTION ON STANDARDS OF TRAINING, CERTIFICATION AND WATCHKEEPING FOR FISHING VESSEL PERSONNEL, 1995, AS AMENDED)

대한민국정부는 정부에 의해서 또는 대신하여 에게 발급된 증서번호 의 면허가 위 개정협약의 제 Ⅰ/7조의 규정에 의하여 해기사자격을 갖추었다는 것과 까지 또는 이 승무자격증의 유효기간에 대한 갱신기간의 만료일까지, 아래에 표시된 면허의 한정을 조건으로 명시된 직무를 수행하기 위한 해기능력을 갖추었음을 증명합니다(The Government of the Republic of Korea certified that certificate No. issued to by or on behalf of the Government of has been found duly qualified in accordance with the provisions of regulation Ⅰ/7 of the above Convention, as amended, and the lawful holder is authorized to serve as specified below, subject to any limitations indicated until or until the date of expiry of any extension of the validity of this endorsement as may be shown above).
이 승무자격증의 소지자는 승무기준에 명시된 다음의 직무로 승무할 수 있습니다(The lawful holder of this endorsement may serve in the following capacity or capacities specified in the applicable safe manning requirements of the Administration).

	주의사항(Remarks) 1. 해기사는 법규를 준수하고 선박항행의 안전과 해양환경의 보전에 노력하여야 합니다(Certificated officers must observe rules and make effort to ensure the safe operation of ships and the preservation of marine environment). 2. 이 승무자격증의 유효기간은 5년이며, 그 유효기간(2쪽의 유효기간 참조)이 만료되기 전에 「선박직원법」 제7조제2항의 규정에 따른 요건을 갖추고 승무자격증의 갱신신청을 하여야 합니다. 갱신신청은 유효기간 만료 12개월 전부터 할 수 있습니다(This endorsement is valid for a period of 5 years and it is required to apply revalidation of endorsement with being met the requirements of the paragraph 2 of Article 7 of Ship Officer's Act prior to its expiry. The application for revalidation is allowed within 12 months prior to the date of expiry of this endorsement). 3. 이 승무자격증은 다른 사람에게 양도하거나 대여할 수 없습니다(This endorsement must not be given or lent to other person). 4. 승무자격증의 취소 또는 업무의 정지처분의 통지를 받은 경우에는 그 통지를 받은 날부터 30일 이내에 이 승무자격증을 지방해양수산청장에게 제출하여야 합니다(In case that a notice of suspension or cancellation of the endorsement was given he or she shall submit his/her endorsement to the Director General of Regional Office of Oceans and Fisheries within 30 days from that date of receiving the notice).
