| 신청서 작성 방법 Instructions | ||||
| 1. 사건과의 관계란에는 고소(고발)인, 피고소(피고발)인, 피의자, 피고인(피고인이었던 자), 피해자, 피진정인, 참고인, 항고인, 피항고인, 재항고인, 피재항고인, ○○○의 가족 등으로 기재합니다. For "relation with case," please fill in plaintiff(accuser), the accused(the informed), suspect, defendant(once defendant), victim, petitionee, witness, appellant, appellee, re-appellant, re-appellee, or family member of XXX. 2. 비고란에는 압수증명 신청 시 압수물품명, 수량, 수색증명 신청시 수색장소, 일시 등을 기재합니다. For certificate of seizure, please fill in the name and the number of seized items in the"Remarks." For certificate of search, please fill in the location and date of search in the "Remarks." 3. 출국가능사실증명의 경우 신청사유란에 출국사유 및 출국지를 기재합니다. For Certification of eligibility to travel(㉒), please fill in the boxes, "Purpose of Visit" and "Destination" 4. ① ~ ㉔ 외의 민원은 별도 신청서에 의하여 신청합니다. Separate forms must be submitted to request certificates and documents other than ① to ㉔. 5. 수수료는 수입인지 또는 수수료 납부 증명서면을 별도의 "수수료납부서"에 첨부하거나 신청서 여백에 부착하여 납부하여야 합니다. The fee should be paid by ways of revenue stamps or proof of payment. For revenue stamps or proof of payment, please attach it to the request form or submit it with a separate fee payment slip.
